Overview

• CFSS (Community First Services and Supports) is a new self-directed program that will replace Minnesota's current Personal Care Assistance (PCA) program. It is expected to be implemented in the coming year. This will provide a new program that will allow people to choose their own workers and purchase items that will help them with their independence. The program will be run by the Minnesota Department of Human Services. It will include training and coaching of job skills, evaluation of job tasks, and direct observation of health-related tasks.

  CFSS is a support program for people with special needs, and it is intended to help people live independently. It will provide participants with personal care assistance in the community. In addition to this, CFSS will provide health-related tasks such as eating, bathing, and taking medication. It will also provide cues that can be used to accomplish tasks. It is part of the Minnesota Department of Human Services (DHS) 2020 Program. In order to participate in CFSS, participants must be Medicaid eligible.

In order to participate in this model, a participant must select a financial management service provider to assist them with budgeting and purchasing goods. This service provider will also help participants manage their budget and ensure compliance with labor laws.

  In order to participate in CFSS, a participant must meet the eligibility requirements and complete an application. They will also need to complete a Service Delivery Plan. The plan should describe how CFSS services will be provided and should be reviewed annually and when a participant's needs change. In addition, the plan should include a worker development plan. A worker development plan will detail how the support worker will be trained.
