Overview

  • CFSS (Community First Services and Supports) is a new self-directed program that will replace Minnesota's current Personal Care Assistance (PCA) program. It is expected to be implemented in the coming year. This will provide a new program that will allow people to choose their own workers and purchase items that will help them with their independence. The program will be run by the Minnesota Department of Human Services. It will include training and coaching of job skills, evaluation of job tasks, and direct observation of health-related tasks.

The Minnesota Department of Human Services (DHS) has put together an interactive video that gives an overview of CFSS. It is available on DHS's website. The video is meant to answer any questions people may have about the program. DHS has also provided a Frequently Asked Questions page that contains more information.

CFSS is a support program for people with special needs, and it is intended to help people live independently. It will provide participants with personal care assistance in the community. In addition to this, CFSS will provide health-related tasks such as eating, bathing, and taking medication. It will also provide cues that can be used to accomplish tasks. It is part of the Minnesota Department of Human Services (DHS) 2020 Program. In order to participate in CFSS, participants must be Medicaid eligible.

The program will be implemented in two models. The first model is the agency model, and will allow a participant to select an agency to be the participant's employer. In this model, participants are allowed to have their spouse or parent serve as a support worker. CFSS participants will also be able to purchase goods that help them with their independence. Participants can also choose to use the funds that they have been allocated to purchase items.

The second model is the budget model. In this model, participants can select an agency to provide CFSS services. In addition, participants can also choose to have a support worker directly employ a worker. Participants will also be allowed to buy goods with their CFSS budget, such as a wheelchair or wheelchair ramp. In order to participate in this model, a participant must select a financial management service provider to assist them with budgeting and purchasing goods. This service provider will also help participants manage their budget and ensure compliance with labor laws.

In order to participate in CFSS, a participant must meet the eligibility requirements and complete an application. They will also need to complete a Service Delivery Plan. The plan should describe how CFSS services will be provided and should be reviewed annually and when a participant's needs change. In addition, the plan should include a worker development plan. A worker development plan will detail how the support worker will be trained.
